To meet Insurance regulations referrals must have an MD or DO as the referring provider.
Please provide complete demographics, insurance information, and reason for referral.
Providers can refer patients to our practice in several ways.

Online Form - you can either upload the referral or fax the required referral notes but will not need to fill out any other documentation.
A secure Provider Portal for your practice which will allow you to send and receive documentation, send messages to our staff for updates, and see the status of your referral in real time. Please call our office at (706)860-6515 if you would like to set up access for your office.
If you anticipate the patient may need a consult after electrodiagnostic (NCS/EMG) testing, please order the consult first. Similarly, if you think the patient will need EMG/NCS, EEG, Carotid dopplers, please order the consult and we will order the specific diagnostic testing for the patient. Please note that outside electrodiagnostic studies will likely need to be repeated due to inability to verify the validity of the outside interpretation. Testing may be ordered without a consultation in appropriate situations.
If the patient is being referred for abnormal imaging please have the patient bring their report as well as imaging on a disc to the visit. Our doctors are able to access online several imaging facilities in Augusta but not all. The doctors like to look at actual images rather than just the reports.
Dr. Shaefer sees ages 12 yrs and up. Dr. Hughes see ages 18 yrs and up.

Our office does not handle Motor Vehicle Accidents, Personal Injury, or file third party insurance.
